I am sort of naive when it comes to these piercings but is 18g the smallest size possible? I have a extremely small hole and it seems like 20g is too big for me.

if a 20 gauge seems to big then 18'wont work. the.bigger the number, the smaller the size. did you have it professionally pierced? normal size is 18

I did have it done professionally with needle over a year ago. I think b/c I used
rings from Claire's my piercing shrank down.

I do tats & piercings professionally in a shop. Standard start for nostril piercing is indeed 18 BUT I have seen others use from 18-22 gauge.... Depending on how long you have had it pierced, you may be able to take the jewelry out to let the opening shrink or stretch the opening slightly to accommodate the next size gauge. Hope this helped :)
