I can see in the pic that the end of the nipple is sealed but is the cap sealed too, so it can't be unscrewed off the bottle? ThankU :)

A reborn baby bottle goes to a reborn. A reborn is a doll that looks VERY realistic I have 2 of them and a lot of people pretend that they are real. Search it up on YouTube or Google it will probably help you understand better(:
