yes, sorry it is a bad picture. The star is in a carrying case to keep it clean in your bag. First tooth brush/gum massager is a separate thing. Looks like a toothbrush. We kept the toddler toothbrush, since we forgot about this set and we skipped first two steps. :-)

It is in a case with the star. It looks like a tooth brush, but it is made out of rubber to massage gums.
