A living will is a legal document that a person uses to make known his or her wishes regarding life prolonging medical treatments. It can also be referred to as an advance directive, health care directive, or a physician's directive.

It is important to have a living will as it informs your health care providers and your family about your desires for medical treatment in the event you are not able to speak for yourself.

What I am offering is a form for a living will.

I am not able to give legal advice. I worked as a legal assistant for over 20 years (for an estate attorney), but I am not an attorney myself.

I am having surgery later this month and since I am married but have a partner of 8 years it is VERY important I designate him in charge of any health decisions should I become unable to speak for myself since we have no legal marriage in place. That is how I use this form. I am putting my boyfriend in charge should critical medical decisions need to be made.

This form requires is your signature and two witnesses.

You will also check off which life saving measures you do and do not want. You will also fill in your name and the name/address of the person you want to be in charge of making any medical decisions.
