My best guess would be up to an 11 lb. baby? Hubby and I make big babies so I never got to put this on my babe ---she was already too big for it at a couple months old. Just to give you and idea my babies weigh 9 + lbs at birth. Hope this helps. I'm assuming the "S" (small) size fits the average sized newborn-3m.....

Yes it's used over a cloth diaper :) ...not sure what you mean by real one---you mean disposable? It's can go over any diaper you so choose. I mean I'd want to cover any kind of dipe with this cute cover--it's adorable--and would add some extra protection :D
