I've not seen the size of the iphone 5. I gave the dimensions so just measure your phone. I believe it should fit though.

I'm not sure. It fits the S4. I gave the dimensions so you can measure your phone and see if it fits. It's made of neoprene and has a little stretch to it.

Yes. The strap is built in and is adjustable. It's just folded in the box.
