No, the Chromecast has a MicroUSB port for power. If your TV (or DVR, or anything else near your TV) has a USB port, you can use the included USB cable to power the Chromecast.

No, the "casting" is free. Pretty much any of those that you can play on your laptop, you can send to your TV. Subscription services like HBO GO, of course, you would need to have an HBO GO account.

It is brand new in sealed box so I'm sure the setup instructions are included. Hulu and NetFlix are pay services so yes, you'd have to pay for those. Cheers.
