it has no service on it now but i bought it from us cellular when i had us cellullar as my provider. is that what u want to know.

Does it take a sim and sd card, wifi enabled, imei clear (no amount to at&t who bought US cellular)
what they mean about locked it means that it is set for that provider only not another

I dont owe anything to at&t if thats what u mean. i thought u could use a cell phone with anyone u just have to put a sim card in it

it has a sim card. I dont know if u have to use us cellular or not wifi i never used internet with it.but pretty sure u can. i believe it was made in 2006. i believe phones were made with the ability to pick up wifi. dont know how else one would be able to have internet.
