Please bare with me as I am pretty computer illiterate. Does this work with any cable modem and is it for dial up modems only? Also, is the cd for connecting instructions included?

It's for phone jack DSL connections only. Not for cable modems. It's not a 'dial up' modem...it's for dsl. Cd is in the box. Hope this helps! Thanks for looking :0)
