I am adding more info/instructions since the photo with instructions is not very clear.
I have not used this, so I am not sure how it works! it has no brand name on it, but it looks just like the Bono mini dent puller that I found on Google.
I purchased this off Groupon, and then saw a larger one that I also bought.
I will say the larger one did pull out a crumpled area in my car's fender pretty well!

here are the instructions I found on Groupon off my order:
