I forgot my iCloud password and no way to retrieve it. My upgrade was up luckily and got an iPhone 6+.

You can call apple and give them the imei and your info and iCloud account and they can reset it if u have all your info to verify that it's your phone been threw it before

Yes, you can go to icloud on your laptop or desktop whatever computer and when it asks you to sign on all you have to do is click on i forgot password then type in your email address and they will ask you 2 questions answer them then you can remove the phone from your account and make sure it;s unlocked!!
