Yes I use one just like it all you have to do is turn it on turn your bluetooth on and search for new devices it will connect then

It came as a bundle with my phone in the same box as my phone so no I don't have a box and I don't know how to tell what model it is, I got my phone new with it in september if that helps any

most bluetooth headsets require a password to connect it to the bluetooth is this the standard 0000 like most? Usually the instructions have it on there.
