This was a gift,I can't wear gold, the preson did not know this at the time. Still in same bag and box when I first got it. It has Dime in the middle and some CZ pretty sure that what on it.

Yes it comes with a chain about a year. It sat in my safe. we moved into smaller house and planned to have a yard sale, but it's to cold here.I found this site so now I'm putting stuff on here, Go to storage everyday and pick through the stuff.
