my question is whats on the other side, not the side you show in picture..Does it have an ad where it came from ( a vet, a dog food company or what) Or is the flip side blank ??

From the picture you would open it up and their is more places to record stuff you get done for your pet.Called Procedures, then under this its called Test Results..On the back when all folded up A place for your Veterinarian`s address and phone number you write in or have your Veterinarian stamp for you.Under this is where your information goes.To the left of this is where you put your Pet`s Information..
Front cover is the Name Health Record and the company who provided this or made this .

I see. The outside cover has the name of the company that provided the record pamphlet and they made spaces for your Pet's information on the left and your Veterinarian's information on the right, of the front cover. So you see these things before you open the pamphlet. That is very good.
