SORRY! IT IS VERIZON. (was looking at current phone). I used Page Plus Cellular though as pre-paid monthly.

Fanned back, and don't know about who all manufactured the brand, but one of the pictures posted shows the back of the phone and it has the Motorola logo and Verizon name. See 3rd from last pic.

It is the cell phone and charger. And it is a Verizon phone (made updated correction). You determine the service provider. If Verizon does not have a good signal in your location, a Verizon phone is not your best bet. However, you can unlock a phone and use any carrier. I used a pre-paid service with this phone to avoid a contract (Page Plus Cellular) and did not go through Verizon. Some phones only work with a contract, some only pre-paid.
