What year was the plate and what company is it from? Does it have any damage? Certificate?

I have updated info. I'm sorry to say that the plate is not in the orginal box of with the certificate. The plate was in my chona hutch and the box with certificate were in the bottom when I had a flood in my house. The plate was protected since it was higher up.

There are no chips or cracks. It sat in a china hutch in non-smoking home. I'm srooy to say the orginal box and certificate were distroyed in a flood.
