yes, I'm sorry, it does use a SIM card and it is included with the phone.

No, this one does not have a camera. Also, I forgot to mention, you can't schedule appointments in it. But it does have a built-in flashlight and a radio that gets most stations and sounds good. I love the radio and flashlight, as I love to walk at night, but I can't have a phone without an appointment alarm. Thats why I got a new one.
