Looks almost like it says Creative on the one chip.

It says CRYSTAL on the chip. On the end it says "GAME" and "MIDI" on it. Hope that helps. Not sure where it says the model on it.

It's a game card, that "Game in" port is for a controller or joystick, what's funny is I have both the joystick and the controller that plug into it lol I'll never use them tho cus I use a Xbox 360 controller now

This is an vintage ISA-BUS (pre-PCI) sound card, not compatible with any modern computer. As a minor of fact it was outdated in 1993 lol
